• Family/Lifetime Activity Passes may be used at DeKalb High School, Clinton Rosette Middle School and Huntley Middle School. Excludes FNBO Challenge games and IHSA Playoff games.

  • Lifetime Family Activity Pass is good for the lifetime of the donor named on this form and is non-transferable.
